Continuing activities and services available for Gates residents:

Integrated, coordinated home health care and related services for older adults who have ongoing health care needs.  Care provided by a team of health care pprofessionals who follow client through all health care settings with the primary purpose of keeping him/her living comfortably at home.

Fees: PRAH accepts Medicaid, medicare, private pay

C.A.R.E. at Home is a unique program that provides seniors and their families with personalized helath evaluation and care coordination services. We can assist with Nursing Home placement including PRI/Screen, Medicaid Application, and Community Care Management.

Area served: Gates, Monroe County

Requirements: chronic health care needs/ home care needs, screening evaluation to determine enrollment eligibility.

Fees: Care and Home varies
